Clinicians recording data in health records, or using the data in these records need to understand the codes used to support computerisation of the meaning intended in a record. They should also understand the requirements for quality representation and how this impacts the design of the health record and clinical decision support information. Clinicians are responsible for patient care and need to be responsible for the information which supports that care. The record is no longer the inanimate legal record, it is the source of handover, decision support, and ongoing care information - and this information must be accurate.
